In 1968, at Hixon level crossing in Staffordshire, a low loader with a 120 tonne transformer aboard was crossing the tracks at less than walking pace when an express train appeared and ploughed into it. Eleven people were killed and 45 injured, and in 1969 these signs - instructing the drivers of large or slow vehicles to phone the signalman for permission to cross - were introduced.
