After many years of designing these I can make an educated guess! The concrete slab is most likely over a rotary screen, to prevent most of the solid matter from reaching the watercourse. The green box no doubt contains the control gear for the screen. Beyond the slab is an array of manhole covers over the storage tank, which reduces the frequency of discharge of storm sewage to the watercourse. I could not find the storm outfall; possibly the flow is piped to the culvert under Bradford Road.
