This tiny one-room chapel (attached to a cottage in the hamlet of Bagshaw, on a no-through road) still hosts services once a fortnight. [Update: according to http://www.derbyshirelife.co.uk/out-about/places/hamlets_in_the_hills_sparrowpit_bagshaw_and_blackbrook_1_1779748 it closed in 2004; it was a private house when I passed last year (2014).]
