To quote the Bereavement Care service notice in http://www.geograph.org.uk/photo/5231454 : "The seat is hexagonal in shape and is made up of six smaller seats. Each smaller seat will have two inscribed markers. Families can lease one or both of the plaques on the seat. Each plaque will display an inscription (message) of your choice in gold lettering. The plaques are available to lease for a period of 5 years."
